If you owe ALLCARE a balance (except if there is a payment plan in place) it is required that all prior balances must be paid in full before your scheduled appointment.
Co-payment and co-insurance must be paid in full BEFORE your scheduled appointment. ALLCARE does not bill for secondary insurance.
If you need a receipt for insurance or tax purposes, a receipt will be provided at the time of service.
Your insurance coverage depends on your individual contract held with your insurance carrier. It is your responsibility to contact our office to insure that authorization for appointments has been obtained. You are bound by your plan to follow their rules and guidelines for obtaining a referral from your primary care physician before being seen in this office. Obtaining a referral is your responsibility and never the responsibility of this office. It is your responsibility to keep track of your referrals and the number of visits they authorize.
Please note that an authorization is not a guarantee of payment by your insurance carrier. ALLCARE is not responsible for notifying you when you are out of authorized visits. Please note that referrals cannot be dated retroactively after the date of service. You remain ultimately responsible for payment of all services. Our staff cannot be familiar with the requirements of all group plans since they can vary considerably within the same plan.
ALLCARE requires all clients maintain a 90% attendance policy. If you receive more than one service and a therapist cancels a session, you are expected to attend the other therapy sessions. Please remember your time slots are reserved exclusively for you.
ALLCARE requires at least 24 hours advance notice for cancellations. If you do not give advanced notice or no show for your reserved time slot, you will be charged a fee.
Please contact the receptionist or leave a message on ALLCARE’s voicemail ASAP. Informing the therapist is not ALLCARE’s cancellation policy.
A fee of $65.00 will be charged for any addition reports or paperwork your insurance company does not cover.
Food and Beverage Policy
Due to severe allergies, please do not bring food or beverages in the waiting room.
In the attempt to minimize the spread of illness to other client’s, families, and staff, please cancel your appointment if you are ill.
In case of inclement weather closing, please listen to Stormwatch 92.7 WOBM, 105.7 HAWK, 1160 WOBM, 94.3 THE POINT and/or log onto www.wobm.com. Also, a message will be left on the answering machine, if the office closes unexpectedly.
The office will not be making individual calls.
ALLCARE will be closed on the following holidays: New Years Eve, New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and the day after, Christmas Eve, and Christmas Day. Any additional closed dates will be posted at the front desk.
